Fax Your Info

Fax the completed order form (and your purchase order if you have one)
to 1-888-349-3437 or e-mail to sales@bridges.com.
Copies of any purchase orders should also be mailed.
Step 3 --
We'll Call to Confirm

A Bridges sales
administrator will call you to confirm your information and check to see
if you qualify for special pricing (such as a volume discount). This
contact will ensure that your order is processed without delay.
Preparing Your
Purchase Order
Complete your order on official letterhead, or a purchase order form, and fax it to us. A copy should also be mailed.
All orders are
subject to the terms and conditions of our site licensing agreement.

Please check that your purchase order
includes the following information:
- Clearly
identifiable purchase order number and billing address
- Contact information for each of the following persons for your site: Ship-to (software kit recipient), Purchaser (if different from Ship-to), main Site Contact and Site Technician. Please include telephone and fax numbers, and an e-mail address for each person (where applicable).
- A signed
Bridges order form, showing the
exact purchase cost including shipping and any applicable taxes.
